Test your Health

One of the most useful steps that you can take to assess and improve your health is to find out where you are mentally and physically relative to the rest of the population. The two best web sites I have found to do this are Attitude Factor and Real Age. These sites are free and easy to use. I highly recommend that you take these tests and encourage your family members to take them. The results will open your eyes as to how well you are doing with your health and what you can do to improve it.

It will take you between 5 minutes and 50 minutes to complete the tests, depending upon how many you choose. The links above will take you to the tests.

Intake Forms

We ask that, at a minimum, you complete a brief western medicine intake form. This enables us understand your current symptoms and past history from a western biomedical symptomic perspective. For more complex issues and for internal medicine conditions, we ask that you also complete a second somewhat longer Chinese medicine intake form. This helps us to more accurately understand your unique health situation and to help us prescribe customized herbal preparations. To save you time when arriving for your first appointment and to help you understand your unique signs and symptoms from both a western medicine and Chinese medicine perspective, you may want to complete both the short intake and long intake forms prior to your visit.

Complete the forms by opening the form ('double clicking') and then placing an "x" next to the appropriate boxes.

How to send the forms back to Steve Paine, OMD

1. Save the form with your added information
2. In Word, there is a "Send" button under the File menu. That will automatically open an email with your completed form as an attachment.
3. Enter your name in the email subject line.
4. Enter info@stevepaineomd.com as the email address and send it
5. Or you can open your email directly and attach the form

Alternatively, you can call us on 2523 8490 and we will fax the forms to you prior to your first treatment session.

We recommend that you complete both intake forms prior to your first visit. We also recommend the educational advantages of reviewing this website prior to your first visit so that you understand something of Chinese medicine and what it may be able to do for you.
